Please Read Carefully – This is a Legal Document

In consideration of being allowed to participate in any way in The Collab and Elite Hoops Academy™ basketball training programs, related events, and activities (hereinafter referred to as "the Program"), the undersigned acknowledges, appreciates, and agrees to the following terms:

1. Acknowledgment of Risk

I acknowledge and understand that participation in basketball training involves risks including, but not limited to, physical injury, strains, sprains, fractures, concussions, heart attacks, or other physical and mental health-related complications. I understand that there may be other risks not known to me or that cannot be foreseen at this time.

2. Assumption of Risk

I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me or my child, or any loss or damage to property owned by me, as a result of participation in the Program, whether caused by negligence or otherwise.

3. Medical Treatment Authorization

In the event of an emergency, I authorize The Collab and Elite Hoops Academy™ to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for the participant's immediate care, and I agree that I will be responsible for payment of any and all medical services required.

4. Release and Waiver of Liability

I hereby release, waive, discharge, and covenant not to sue The Collab, Elite Hoops Academy™, their owners, directors, coaches, employees, volunteers, or agents from any and all liability, claims, demands, actions, and causes of action related to any loss, damage, or injury, including death, that may be sustained by me or my child, or any property belonging to me, arising out of or related to participation in the Program.

5. Indemnification

I agree to indemnify and hold harmless The Collab, Elite Hoops Academy™, their employees, coaches, agents, and volunteers from any loss, liability, damage, or costs, including court costs and attorney’s fees, that may occur due to my or my child’s participation in the Program.

6. Photo and Media Release

I understand that The Collab and Elite Hoops Academy™ may take photographs and videos of the participants during training sessions and events. I grant permission for The Collab and Elite Hoops Academy™ to use these images for promotional purposes, including but not limited to social media, website, and other marketing materials.

7. Agreement to Follow Program Rules

I agree that my child and I will comply with all Program rules, policies, and directions provided by coaches, staff, or volunteers of The Collab and Elite Hoops Academy™. I understand that failure to do so may result in termination of participation without refund.

8. Severability

If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable.

9. Governing Law

This waiver shall be governed by and construed in accordance with the laws of the state of Texas.
